Community-based Diabetes and Hypertension Preventive Program

An Evidence-Based Practice

Description

This program was developed to provide diabetic and hypertension programs in Chinatown. Seventy-five Chinese individuals with either Type 2 diabetes, hypertension, or both participated in the program. All of the members volunteered to have their blood pressure and/or blood glucose levels measured at baseline and biweekly visits for one year. All of the members consulted with a diabetes nurse educator for an individualized meal plan, exercise plan, preventive plan for hyperglycemia and hypoglycemia, and foot care. The meal plans and exercise plans were developed based on the individual’s cultural food preference with mutual short term goal setting at the beginning of the project .

Goal / Mission

The purpose of this project was to develop rapport with a Chinese Community Association and then establish preventive diabetic and hypertension programs with the Chinese in Chinatown, Hawaii.

Results / Accomplishments

There was a significant decrease in mean blood glucose levels among participants in the preventive program (p=<0.001). There was also a significant reduction observed in the mean systolic and diastolic blood pressure before and after the program(p=<0.05).
