The Healthy Mendocino Community Healthy Improvement Plan (CHIP), is a collaborative effort among citizens and organizations to improve local health and the factors that influence it. The CHIP builds on the Community Health Needs Assessment (CHNA), completed in early 2016.

For the 2016-2019 CHIP cycle, action teams were formed around the five priority areas of Mental Health, Poverty, Childhood Trauma, Family Wellness and Childhood Obesity and Housing. Teams were formed for both the Inland and North Coast regions. Healthy Mendocino staff provided guidance and support in helping the teams reach their goals.
