CHIP Action Team SMART Goals


CHILDHOOD OBESITY/FAMILY WELLNESS UKIAH AND NORTH COAST

Goal 1: Increase the number of 7th grade students who are physically fit from 59.7% to 63% county wide based on Aerobic Capacity through supporting safe and secure walking and biking routes and trails, hosting signature community events aimed at promoting exercise by July 2020.
Goal 2: Increase the number of 5th grade students who are in the Healthy Fitness Zone for weight from 52.4% to 56%. (Body Composition) through promoting healthy environments at workplace, recreation, and community meeting venues and supporting efforts of agencies and organizations to promote physical activity and healthy nutrition of children and families by July 2020.
Goal 3: Implement one workplace environmental change to promote nutrition and healthy weight in 75% of businesses surveyed by July 2020.


UKIAH POVERTY ACTION TEAM

Goal 1: Complete surveys of at least 125 households living in poverty in Mendocino County to better understand what poverty looks like in Mendocino County through data collection and community focus groups by March 2019.
Goal 2: Hold a Poverty Summit involving the participation of policy makers, program operators, educators, community organizations and other key stakeholders in Mendocino County in October 2018.
Goal 3: Generate at least four different initiatives to improve the living conditions of (1) children, (2) families, (3) single individuals, and (4) seniors with measurable outcomes by 2020.


NORTH COAST POVERTY ACTION TEAM

Goal 1: Support community members in gaining financial success and economic independence through 4 money management workshops and small business management trainings a year by July 2020.
Goal 2: Have financial counselors available to provide direct services for one on one financial consultations for youth, families, and senior citizens in prominent agencies serving those populations by July 2020.


UKIAH HOUSING ACTION TEAM

Goal 1: Hold two policy training workshops for HAT members to learn about the development of and regulations around Accessory Dwelling Units (ADU) and Smart Growth by July 2020.
Goal 2: Organize at least two public education workshops and programs to identify barriers to afford housing; improve community knowledge about housing and development opportunities; and provide housing boot camp to help people interested in building an ADU by July 2020.
Goal 3: Send at least five (5) letters to City Council members and Supervisors regarding proposed regulatory and policy changes 4 times a year to influence housing planning and community  development through July 2020.
Goal 4: Write at least three (3) letters to the local newspaper editors once a year promoting viable housing projects through July 2020.


NORTH COAST HOUSING ACTION TEAM

Goal 1: Influence two or more “housing positive” changes in County and City policies and regulations by sending at least five letters to City Council members and Supervisors with proposed and regulatory and policy changes regarding market affordable housing by July 2020.
Goal 2: Attend six city council meetings to contribute to and influence policy and decisions on the development of “housing positive” policy in the Mill Site Reuse Plan & Local Coastal Plan Amendment that result in affordable and diverse types of housing by July 2020.
Goal 3: Organize three public education workshops and programs to identify barriers to affordable housing; improve community knowledge about housing and housing development opportunities; and provide housing boot camp to help people interested in building an ADU increasing the number of ADUs constructed in Fort Bragg from 3-5 per year to 7-10 per year by July 2020.
Goal 4: Publish three articles a year on housing and make 10 presentations to local community, professional, social services and health organizations by July 2020.


UKIAH MENTAL HEALTH ACTION TEA


Goal 1:
Compile an online directory of mental health resources for mental health providers and the community through the Healthy Mendocino website by July 2020.


NORTH COAST MENTAL HEALTH ACTION TEAM


Goal 1:
Increase outreach to the regional middle-school and high school suicide prevention programs and literature by 30% by July 2020.
Goal 2: Increase education and information about suicide prevention to the regional senior population by 20% by July 2020.
Goal 3: Inform the community about suicide prevention through resources and support services through public events, literature, radio interviews, television, and social media reaching all sectors of the population by 40% by July 2020.
Goal 4: Provide information and action plans that address issues of mental health and support continuum of care and case management to address suicide prevention to 75% of the north coast community by July 2020.

UKIAH CHILDHOOD TRAUMA TEAM

Goal 1: Facilitate at least six longer trainings (more than four hours) focused on trauma-informed communities, child abuse prevention, ACEs, and resiliency throughout the county by July 2020.
Goal 2: Develop a short (less than 20 minutes) intro to ACEs presentation that is locally-specific and can be incorporated into a variety of existing meetings across a wide variety of settings by July 2020.
Goal 3: Develop and distribute new posters for the Child Abuse Awareness walkabout by April 2019.


NORTH COAST CHILDHOOD TRAUMA ACTION TEAM

Goal 1: Develop trauma-focused resource materials for parents, agencies, and the general community on the north coast including a resource map for working with agencies by July 2020.
Goal 2: Offer two workshops a year on the prevention of and intervention for childhood trauma and building resiliency for families and professionals through July 2020.
Goal 3: Organize and create one teacher ongoing support group and one parent ongoing support group for practicing resiliency skills on the north coast using Connection-Based Techniques by July 2020.
Goal 4: Attend three local government and county government meetings a year to speak about and inform government officials of ongoing efforts to decrease childhood trauma through July 2020.
Goal 5: Offer self regulation skills training for teachers, youth advocates and school counselors through an evidence based self-regulation and resiliency skills curriculum to serve at least 50% of north coast youth ages 12-18 by July 2020 .
